Most doctors just order the TSH and Total T4 which I explained above, are useless. It is therefore paramount that you print out and take the checklist with you to the doctors office and insist on getting the full panel. What Is The Difference Between T4 And Ft4

T4 is the measure of thyroxine hormone circulating in the bloodstream. It is the sum total of both bound and free T4 hormones. FT4 is a measure of free T4 hormones that remain in the unbound state and can thus enter the body cells and affect the body tissues. The T4 reading along with TSH provides the most accurate picture of your thyroid gland.

How Does The Thyroid Gland Function

The major thyroid hormone secreted by the thyroid gland is thyroxine, also called T4 because it contains four iodine atoms. To exert its effects, T4 is converted to triiodothyronine by the removal of an iodine atom. This occurs mainly in the liver and in certain tissues where T3 acts, such as in the brain. The amount of T4 produced by the thyroid gland is controlled by another hormone, which is made in the pituitary gland located at the base of the brain, called thyroid stimulating hormone . The amount of TSH that the pituitary sends into the bloodstream depends on the amount of T4 that the pituitary sees. If the pituitary sees very little T4, then it produces more TSH to tell the thyroid gland to produce more T4. Once the T4 in the bloodstream goes above a certain level, the pituitarys production of TSH is shut off. In fact, the thyroid and pituitary act in many ways like a heater and a thermostat. When the heater is off and it becomes cold, the thermostat reads the temperature and turns on the heater. When the heat rises to an appropriate level, the thermostat senses this and turns off the heater. Thus, the thyroid and the pituitary, like a heater and thermostat, turn on and off. What Is Reverse T3

What Are Optimal Thyroid Levels?

Reverse T3 is the inactive form of the actual T3 hormone. It has the same structure as that of T3 and the only difference is the position of the iodine atom in the molecular structure. This factor makes it biologically inactive. Measurement of this thyroid hormone is not considered clinically useful.

How Is The Thyroid Profile Test Performed

The thyroid profile test is performed by a simple venipuncture procedure. Here, a blood sample is collected from the arm. A tourniquet or elastic band is tied across the upper arm tightly as this increases the pressure of the blood in the veins. This makes it easy to locate veins. Once the vein has been located, a syringe is slowly injected and blood is drawn from this vein. The blood gets collected in a vial. Once the blood has been drawn, the syringe is slowly withdrawn and a sterile gauze is used to cover the puncture site to prevent bleeding and infection. The sample is sent to the lab for testing.

What Is A Triiodothyronine Test

This test measures the level of triiodothyronine in your blood. T3 is one of two major hormones made by your thyroid, a small, butterfly-shaped gland located near the throat. The other hormone is called thyroxine T3 and T4 work together to regulate how your body uses energy. These hormones also play an important role in controlling your weight, body temperature, muscle strength, and nervous system.

The T3 hormone comes in two forms:

  • Bound T3, which attaches to protein
  • Free T3, which does not attach to anything

A test that measures both bound and free T3 is called a total T3 test. Another test called free T3 just measures free T3. Either test may be used to check T3 levels. If T3 levels are not normal, it can be a sign of thyroid disease.

What Does The Test Measure

The TSH test measures thyroid-stimulating hormone, which is a hormone that prompts the thyroid to produce other hormones. The thyroid is a butterfly-shaped gland in your neck that makes the hormones triiodothyronine and thyroxine . T3 and T4 control your metabolism, or how your body uses and stores energy.

Although TSH acts upon the thyroid gland by binding to the TSH receptor, it is made in the pituitary gland. Reason #: T3 Hormone Is King

T3, also known as triiodothyronine, is the hormone that our body cells have receptors for. This means that for you to have beautiful hair, healthy nails, optimal gut health, a sharp mind and good mood, you need a sufficient amount of the T3 hormone, not T4.

Interestingly, the T3 hormone is produced from T4 by a metabolic conversion that happens in the gut and the liver. Can you see now how you could have a sufficient T4 hormone but if your T3 is low, you will still be feeling like a zombie?

Oh, and by the way, Synthroid is a synthetic version of T4 this means that you still need to convert it to T3 to feel good.

What Initial Tests Are Used To Diagnose Hypothyroidism In Dogs

Thyroid hormones have widespread effects on the body, and are involved in the metabolism of food as well as the daily metabolic functions of most of the body’s tissues and organs. Therefore, the effects of decreased thyroid hormone concentrations may be seen in many initial blood and urine screening tests. Changes in the initial screening tests help identify the presence of hypothyroidism, and may uncover other conditions, some of which may be related to decreased thyroid hormone production while others may indicate other, non-related problems.

Complete blood count . The white blood cells, red blood cells, and platelets are evaluated in this test. Occasionally a mild anemia may be seen with hypothyroidism because thyroid hormone is required for optimal red blood cell production. Changes in the appearance of the individual red blood cells may also be noted. While target cells are not specific for the diagnosis of hypothyroidism, their presence is supportive of the diagnosis.

Hypothyroidism may result in compromised immune function, so occasionally an increased number of white blood cells may be found because of some concurrent inflammatory or infectious condition.

What About Those Already On Thyroid Medications

If youre already on thyroid meds, but still have symptoms, you may need to up your dosage so that your labs fall within the optimal range, which in turn can help you feel better.

Please take a look at my article on TSH for a letter that you can take to your physician if he/she is not familiar with the current optimal reference range. As I mentioned earlier, I personally feel best with a TSH a bit under 1IU/mL, but you may need some trial and error to find your personal best TSH.

Once you establish a dose of medication thats working for you and as long as your symptoms dont change you can test your thyroid hormones every six months.

How To Get Tested

Thyroid Testing

A thyroid panel requires a blood sample. Laboratory testing involves using a needle to remove a small amount of blood from a vein in your arm. The procedure can happen in the doctors office, hospital, or another laboratory.

In most cases, a thyroid panel is ordered by a doctor. Sometimes the thyroid panel analysis is automatically done by the lab on the original blood sample if an initial evaluation finds abnormal levels of thyroid-stimulating hormone.

Why You May Need A Thyroid Test

Women of all ages are more likely than men to have low thyroid hormone levels. However, many of their symptoms are attributed to other conditions or written off as a consequence of aging.

A blood test for levels of TSH is the most sensitive test for determining whether you have hypothyroidism. Most laboratories use 0.45 5.00 mIU/L as a normal reference range for TSH. People with TSH between 5.00 and 9.99 mIU/L often have no symptoms , but some do. Another test called T4 will be done if your TSH is in this range. A low level of T4 usually means you will benefit from thyroid hormone replacement.

Many people with hypothyroidism or subclinical hypothyroidism aren’t aware anything is wrong because they haven’t been tested. The U.S. Preventive Services Task Force doesn’t recommend thyroid screening because it hasn’t found sufficient evidence that testing thyroid hormone levels in large groups of people without symptoms is cost-effective. Dr. Garber agrees, and suggests a different approachtesting asymptomatic people who are most likely to develop thyroid disease and benefit from treatment.

Treating subclinical hypothyroidism with synthetic thyroid hormone may reduce the risk of developing more serious problems like cardiovascular disease. They note that low thyroid hormone can cause a high cholesterol level and treatment with thyroid hormone may make statin therapy unnecessary.

What Is The Significance Of Tsh Reading

TSH provides the best and most accurate picture of your thyroid glands functioning. The level of TSH is usually treated as an early warning system for thyroid disorders. High TSH indicates that the thyroid gland is not creating enough thyroid hormones and is indicative of an underactive thyroid or hypothyroidism. Conversely, a low TSH means too much thyroid hormone secretion or hyperthyroidism. Occasionally, a low TSH may result from an abnormality in the pituitary gland, which prevents it from making enough TSH to stimulate the thyroid . In most healthy individuals, a normal TSH value means that the thyroid is functioning properly.

What Are Thyroid Nodules

Thyroid nodules are small bumps that can form inside the thyroid gland. Approximately 90% of thyroid nodules are non-cancerous however, in some cases, these nodules may suggest the presence of thyroid cancer. Also, thyroid nodules can sometimes alter thyroid hormone levels, resulting in either hypothyroidism or hyperthyroidism.

Women are about 4 times more likely than men to have thyroid nodules. And older age, a history of thyroid radiation, and iron deficiency are all linked with a greater likelihood of having thyroid nodules.

Antithyroid antibodies are specific antibodies created by the bodys own immune system that attack and destroy healthy thyroid cells. This is treated as the bodys autoimmune response to healthy body cells. This factor is considered responsible for Hashimotos disease and Graves disease. These antibodies target critical points of the thyroid gland and impact the levels of thyroid peroxidase, thyroglobulin, and TSh receptors. The additive effect is abnormal thyroid hormone secretion which can lead to an overactive thyroid or an underactive thyroid.

What Is The T4 Hormone

The T4 test and the TSH test are the two most common thyroid function tests. Theyre usually ordered together. The T4 test is known as the thyroxine test. A high level of T4 indicates an overactive thyroid . Symptoms include anxiety & panic attacks, sudden weight loss, tremors, and diarrhea. Most of the T4 in your body is bound to protein. A small portion of T4 is not and this is called free T4. Free T4 is the form that is readily available for your body to use. Sometimes a free T4 level is also checked along with the T4 test.

What You Can Do

If you have definite symptoms of hypothyroidism, you should talk to your clinician about being tested. If your LDL cholesterol has been advancing or your weight has been creeping up unexplainably, you’ll want to discuss getting a test. If you’re 60 or older and generally healthy, it’s still a good idea to check with your doctor to see whether your medical history suggests you might benefit from testing.

What Do The Results Mean

High TSH levels can mean your thyroid is not making enough thyroid hormones, a condition called hypothyroidism. Low TSH levels can mean your thyroid is making too much of the hormones, a condition called hyperthyroidism. A TSH test does not explain why TSH levels are too high or too low. âThe pituitary gland produces thyroid-stimulating hormone in the brain. The primary job of TSH is to regulate the amount of thyroid hormonesâthyroxine and triiodothyronine âproduced by the thyroid gland. This blood test measures how much overall thyroid-stimulating hormone is in your blood. While this is the standard test most doctors authorize, it won’t provide detailed information on your thyroid activity. For instance, it may not show problems like your body not being able to use the available thyroid hormone.

Free T4

The thyroid produces two hormones: thyroxine and triiodothyronine . T4 is primarily inactive. Its primary job as a “storage hormone” is to transport T3 to the proper organs in your body. T4 must be converted to T3 before your body can use it to get energy or deliver oxygen to cells. The T4 test helps determine the level of thyroid hormone in the body however, it doesn’t provide enough information by itself because it can be affected by the amount of protein in your blood.
