Cpt Code Updates For Fine Needle Aspiration Biopsy

Fine Needle Aspiration is a simple biopsy technique in which a thin needle is passed through the skin to obtain a sample of a fluid or tissue from a swelling or lump. In 2019, there are several changes to FNA CPT codes. Partnering with an experienced medical billing and coding company is the best way for physicians to keep up with these updates and report FNA procedures for optimal reimbursement.

FNA A Simple Office Diagnostic Procedure

FNA is usually done in the breast, thyroid gland or lymph nodes in the neck, groin, or armpit. It is an effective technique for rapid tissue diagnosis of a suspicious lump, cyst or mass discovered in these areas during a physical exam, CT scan, mammogram or ultrasound. A relatively non-invasive, less painful and quicker method than surgical biopsy, FNA can help make a diagnosis or rule out conditions such as cancer. This simple office technique which is performed through the skin comes with many benefits:

    • Rapid, highly accurate diagnosis
    • Samples can be taken from various sites in one sitting
    • Minimal discomfort for the patient
    • Makes possible many ancillary techniques such as bacterial culture, flow cytometry, cytogenetics, etc.
    • Safe with low rate of complications
    • Requires little recovery and results are available quickly

Definition Of Professional Versus Technical Fees/payments For Ultrasound Cpt Codes And Reimbursement

We hear Professional versus Technical fees and payments all of the time. It may seem confusing sometimes but its actually pretty simple. I want to go over the differences between them and some ultrasound coding guidelines before you start looking at the CPT code list.

Professional Fees or Payment of a charge covers the cost of the PHYSICIAN or PROVIDERs professional services. Basically you are charging for your time and expertise for performing the ultrasound exam. For the most part, you should be able to bill for this, assuming you have a process to archive and QA/QI your scans. The Professional Fee/Payment will be received by your physician group directly.

The Technical Fees or Payment of a charge addresses the use of the equipment, facilities, non-physician medical staff, supplies, etc. Basically it is the fees for the equipment cost. Usually, this payment will go to the hospital or institution that purchased the ultrasound machines. Unfortunately, many institutions dont realize the importance of billing for technical fees and only bill for professional fees. However, weve found in our practice that billing for technical fees is very important when it comes time to purchase new ultrasound machines. If you can show the hospital that you are generating revenue for them through technical fees it is much easier to get them to put new ultrasound machines in the budget!

How to report ultrasound with ultrasound guided biopsy of thyroid

QUESTION: I am looking for clarification on the following scenario: A patient presents with two scripts, one for an ultrasound of the thyroid and one for an ultrasound guided biopsy of the thyroid. Is it appropriate to charge for both the ultrasound of the thyroid and ultrasound guidance during the same visit? If yes, should I append a modifier? If the physician takes two biopsies, is it appropriate to charge CPT 76942 x 2?

ANSWER: Let’s start with the two requisitions for the tests, based on the scenario you present. A diagnostic implication must justify the first requisition for the ultrasound of the thyroid. Use CPT code 76536 on the same day as a scheduled ultrasound guided biopsy of the thyroid.

Based on the second requisition–ultrasound guided biopsy of the thyroid–with appropriate medical record documentation, you may report 76942, Ultrasonic guidance for needle placement , imaging supervision and interpretation, as well as 60100, Biopsy thyroid, percutaneous core needle.

Although the physician performed two biopsies, only report one CPT code for the biopsy, regardless of the number of biopsies the physician performs on the same lesion. If the documentation supports separate and distinct lesions biopsied in each lobe of the thyroid, report code 60100 twice.

What Does The Equipment Look Like

The needle used is a thin, fine-gauge needle that is smaller in diameter than the needle used in most blood draws . The aspiration may be done with a needle or with a needle that is attached to a syringe. The syringe may be in a plastic or metal holder to make it easier for the doctor to aspirate the cells.

Ultrasound is used to guide accurate placement of the needle within the thyroid nodule.

Ultrasound scanners consist of a computer console, video display screen and an attached transducer. The transducer is a small hand-held device that resembles a microphone. Some exams may use different transducers during a single exam. The transducer sends out inaudible, high-frequency sound waves into the body and then listens for the returning echoes. The principles are similar to sonar used by boats and submarines.

The technologist applies a small amount of gel to the area under examination and places the transducer there. The gel allows sound waves to travel back and forth between the transducer and the area under examination. The ultrasound image is immediately visible on a video display screen that looks like a computer monitor. The computer creates the image based on the loudness , pitch and time it takes for the ultrasound signal to return to the transducer. It also takes into account what type of body structure and/or tissue the sound is traveling through.

Fine needle aspiration is performed with a very fine gauge needle usually from 25 to 27 gauge 1.5 inch. The needle is thin and the needle gauge is smaller is diameter than the normal needles. Now, this is a minimal invasive procedure, hence to perform this procedure we also used guidance. Ultrasound guidance is generally used in Fine needle aspiration of thyroid.

Nodules or abnormalities in the body are often detected by imaging examinations. However, it is not always possible to tell from these imaging tests whether a nodule is benign or cancerous.

The ultrasound guidance helps in the proper positioning of the needle in the target area. Hence, when there is a thyroid nodule and fine needle aspiration is performed, the needle is placed percutaneously in the nodule under ultrasound guidance. In, Medical coding, the CPT code for Fine Needle aspiration of thyroid is 10021 and 10022 . The difference is only about with or without guidance.

    What Is Ultrasound

Fine Needle Aspiration Thyroid Nodule (graphic)

Ultrasound imaging or sonography, involves exposing part of the body to high-frequency sound waves to produce pictures of the inside of the body. Ultrasound exams do not use x-rays. Because ultrasound images are captured in real-time, they can show the structure and movement of the body’s internal organs, as well as blood flowing through blood vessels.

Ultrasound imaging is a noninvasive medical test that helps physicians diagnose and treat medical conditions.

Doppler ultrasound is a special ultrasound technique that evaluates blood flow through a blood vessel, including the body’s major arteries and veins in the abdomen, arms, legs and neck.

Ultrasound scanners consist of a console containing a computer and electronics, a video display screen and a transducer that is used to scan the body and blood vessels. The transducer is a small hand-held device that resembles a microphone, attached to the scanner by a cord. The transducer sends out high frequency sound waves into the body and then listens for the returning echoes from the tissues in the body. The principles are similar to sonar used by boats and submarines.

The ultrasound image is immediately visible on a nearby video display screen that looks much like a computer or television monitor. The image is created based on the amplitude , frequency and time it takes for the sound signal to return from the patient to the transducer and the type of body structure the sound travels through.

    How To Get Tested

A thyroid panel requires a blood sample. Laboratory testing involves using a needle to remove a small amount of blood from a vein in your arm. The procedure can happen in the doctors office, hospital, or another laboratory.

In most cases, a thyroid panel is ordered by a doctor. Sometimes the thyroid panel analysis is automatically done by the lab on the original blood sample if an initial evaluation finds abnormal levels of thyroid-stimulating hormone.

Can I Take The Test At Home

At-home thyroid test kits are commercially available without a prescription and measure the same hormones as the traditional thyroid panel.

For at-home testing, you prick your finger to get the necessary blood sample. That sample is then mailed to a laboratory that conducts the thyroid panel and makes the results available to you directly.

While at-home testing can measure thyroid levels, it is generally not a substitute for a test ordered by your doctor. If an at-home test detects abnormal thyroid levels, your doctor will likely recommend a new blood sample and thyroid panel to confirm the results. At-home tests for thyroid labs may not be as accurate as tests done in a laboratory.
