  • High TSH, normal T4, normal T3 = This is the pattern we’ve been discussing in this post and will usually present with hypothyroid symptoms.
  • High TSH, normal T4, low T3 = This pattern may be an indication that you are having issues with thyroid conversion and most often presents with hypothyroid symptoms.
  • Normal TSH, low T4, low T3 = This pattern is usually what is seen in those with chronic illness and in those who are taking multiple medications. If you have this pattern you will most likely be symptomatic.
  • Normal TSH, normal T4, low T3 = This pattern may be consistent with low T3 syndrome or euthyroid sick syndrome and may present with hypothyroid symptoms but not always.
  • Normal TSH, normal T4, normal T3, positive thyroid antibodies = This is a classic presentation for early Hashimoto’s and most patients with this pattern will be symptomatic. If you are symptomatic this is a valid reason to consider a trial of thyroid medication even though your lab tests are “normal”.
  • Normal TSH, normal T4, normal T3, high reverse T3 = This pattern is most often seen immediately after calorie restriction or after dieting and weight loss. This pattern usually indicates an adaptive response from your body and one that shows your metabolism will be slowing over the next few months .

In People Being Treated For Thyroid Disease

A high TSH may be found in people being treated for either hypo- or hyperthyroidism.

With hypothyroidism, a high TSH usually means that the dose of thyroid hormone replacement needs to be increased.

In some cases, however, the dose is OK, but the medication is not being fully absorbed. For example, many foods and medications can affect the absorption of Synthroid in your system. Learning what to avoid while taking this medication is important for it to work properly.

What Happens If I Have Too Little Thyroid Stimulating Hormone

If a person has too little thyroid stimulating hormone, it is most likely that their thyroid gland is making too much thyroid hormone, that is, they have an overactive thyroid or hyperthyroidism, which is suppressing the thyroid stimulating hormone. People with an overactive thyroid have the opposite symptoms to those with hypothyroidism, i.e. they lose weight , feel too hot and can experience palpitations or anxiety. They may also have a slightly enlarged thyroid gland. Treatment is medication in the form of tablets, which reduce the activity of the thyroid gland and return all thyroid hormone levels to normal. Rarely, problems in the pituitary gland can also result in a low thyroid stimulating hormone, and low free thyroid hormone levels.

What Do The Results Of Tpo Antibodies Test Mean

The TPO antibodies test results may show one of the following options:

  • Negative as you can already conclude negative TPO antibodies test result indicates that no thyroid antibodies were found. This result also implies that thyroid problems and symptoms a patient experiences are not caused by an autoimmune condition
  • Positive antibodies to TPO and/or Tg this result indicate a patient may have Hashimotos thyroiditis
  • Positive antibodies to TPO and/or TSH may indicate the presence of Graves disease

When it comes to TPOAbs, the reference value is < 9.0 IU/ml . Values above 9.0 are typically associated with autoimmune thyroid disease, but elevations are also observed in some other autoimmune conditions. The most commonly mentioned TPOAb reference range is less than 35 IU/ml.

The presence of TPOAbs in patients with subclinical hypothyroidism predicts an elevated risk of overt hypothyroidism. Moreover, this also shows that those patients could be at a higher risk of developing other autoimmune conditions like type 1 diabetes.

What Is A T4 Test

T4 Tests A Total T4 test measures the bound and free thyroxine hormone in the blood. A Free T4 measures what is not bound and able to freely enter and affect the body tissues.

What does it mean if T4 levels are abnormal? Importantly, Total T4 levels are affected by medications and medical conditions that change thyroid hormone binding proteins. Estrogen, oral contraceptive pills, pregnancy, liver disease, and hepatitis C virus infection are common causes of increased thyroid hormone binding proteins and will result in a high Total T4. Testosterone or androgens and anabolic steroids are common causes of thyroid hormone binding proteins and will result in a low Total T4.

In some circumstances, like pregnancy, a person may have normal thyroid function but Total T4 levels outside of the normal reference range. How Does The Thyroid Gland Function

When the heat rises to an appropriate level, the thermostat senses this and turns off the heater. Thus, the thyroid and the pituitary, like a heater and thermostat, turn on and off.
