Camilles story Hashimotos & Vitiligo
One morning as I stepped out of the shower to dry off I noticed small white patches on my hands and arms.; At first, I thought my eyes were playing tricks on me but upon further inspection, I noticed them in other areas of my body as well.; They seemed to have come out of nowhere and I assumed that perhaps I was dealing with some sort of topical fungal infection.
I finished my morning routine and decided to consult my virtual doctor, Dr. Google.
Not to toot my own horn but I am a skilled researcher and after some digging around on the internet, I came across information that matched my mysterious skin condition, and it had an exotic-sounding name.
Vitiligo.; This was a term I wasnt familiar with and I began to wonder if this was yet another co-condition of Hashimotos which I had been struggling with for many years.
In Camilles case and after reviewing the research I wondered which came first for her Hashimotos or vitiligo since it can take many years for anti-thyroid antibodies to appear positive on a lab test.; Since the outward signs of vitiligo are obvious, I can see why Camille thought her vitiligo condition came after Hashimotos.

What Is The Thyroid
The thyroid gland is a small butterfly-shaped organ located just below the Adams apple, or larynx. Its job is to take iodine from foods and convert it into two major thyroid hormones:
Triiodothyronine : The active thyroid hormone
Thyroxine : The thyroid storage hormone
These two hormones circulate throughout the body and regulate the metabolism of every cell in the body. If the level of the thyroid hormones drops too low, the pituitary gland produces thyroid stimulating hormone to stimulate the thyroid gland to produce more hormones. If T3 and T4 levels are too high, the pituitary gland releases less TSH to slow production of these hormones.

Reporter Gene Assay And Cell Culture
The FoxD3 promotor was amplified from wild type genomic DNA by PCR and cloned into the pGL3 basic vector . The novel variant and the -639 T variant were generated by site-directed mutagenesis. Positive clones were verified by sequencing.
Human teratocarcinoma NTERA-2 and embryonic kidney Hek293 cells were transfected with Lipofectamin 2000 . Human acute T cell leukemia Jurkat cells were transfected via microporation using NeonTM Transfection System .
Prior to transfection, adherent cells were seeded into 12-well-format at a density of 6 × 104 cells and 2 × 105 cells per well. Suspension cells were directly taken from culture flasks and applied at a density of 4 × 105 cells per transfection. Cells were transfected with 1.6 g reporter plasmid or pGL3-vector control plasmids plus 0.16 g Renilla luciferase plasmid for NTERA-2 and 0.08 g for Hek293 and Jurkat. Cells were collected 24 hours after transfection and lysed with Passive Lysis Buffer . Activities of firefly and Renilla luciferase were sequentially measured for each sample with a luminometer . Luciferase values were normalized for differences in transfection efficiency as measured by renilla and divided by luciferase values obtained for pGL3-vector control. Luciferase activity of WT reporter plasmid was set 100%. All data represent mean values of at least three independent experiments performed in triplicate.
What Are The Most Common Thyroid Problems

Background. Vitiligo is a common skin disorder characterized by macular depigmentation of the skin. The etiopathogenesis of the disease is still unclear, but there is evidence that autoimmunity and endocrine disfunction may be involved. Objective. The aim of this study was to determine whether vitiligo is statistically associated with thyroid autoimmunity. Method. In a prospective case-control study, we compared the frequency of thyroid autoantibodies in 33 patients with vitiligo and in 33 healthy volunteers. Thyroid autoantibodies and thyroid hormones , triiodothyronine , and thyroid stimulating hormone were measured in all subjects. Results. Thyroid functional abnormalities were found in 6 patients. Anti-Tg and anti-TPO were positive in 9 and 8 patients, respectively. In control group, only one subject had abnormalities in thyroid hormonal status, and two subjects had positive thyroid autoantibodies. Compared with the control group, the frequency of both anti-Tg and anti-TPO was significantly higher in those with vitiligo . . This study shows a significant association between vitiligo and thyroid autoimmunity, and that tests to detect thyroid autoantibodies are relevant in patients with vitiligo.

Diagnostic Tests For Autoimmune Thyroid Dysfunction
- Free T3: Measures blood level of T3 hormone that is unbound, meaning that it is available;to be used by cells and tissues. OR;T3 : When not preceded by the word Free, it is referring to the Total amount, which includes both bound and unbound hormones.
- Free T4: Measures blood level of T4 hormone that is unbound, meaning that it is available to be used by cells and tissues.
- TPO test: Commonly used to detect autoimmune thyroid disease by identifying antibodies directed against TPO.
- TSH test: Measures the level of thyroid stimulating hormone in the blood, ordered when signs or symptoms of an overactive or underactive thyroid condition is suspected or diagnosed, or to monitor for those conditions.
Free T4 vs T4:;The total T4 test can be affected by the amount of protein in the blood that is available to bind to the hormone, as opposed to the free T4, which is not affected by the protein levels. The free T4 test is considered to be a more accurate reflection of thyroid hormone function and in most cases, its use has replaced the total T4 test.
Can Thyroid Medications Cause Acne
Dr. Mikhael says that thyroid medications dont typically cause acne either unless youre allergic to an ingredient they contain. But some acne medications can affect thyroid tests. For example:
- Retinoids: Certain types of retinoids can cause abnormalities in your thyroid-stimulating hormone .
- Isotretinoin: We sometimes prescribe oral isotretinoin for severe acne. It may cause alterations in thyroid function tests, including higher TSH and lower levels of the thyroid hormones T3 and T4.
So now that your thyroid is off the hook for causing your acne breakouts, what IS the cause?
It could be several things changing hormone levels, makeup products you use, a medication side effect, stress or simply too much oil. Cleaning your face twice a day, putting on proper moisturizing lotions and avoiding pore-clogging makeup should help prevent it, suggests Dr. Mikhael.

The Vitiligo Thyroid Connection
Vitiligo is a skin condition characterized by a loss of pigment in some areas of the skin. It can be limited to a certain area or more widespread. It is an autoimmune condition like Hashimotos thyroiditis and Graves disease. Even though patients with vitiligo have a genetic predisposition, environmental factors might be triggers or activating factors of the condition.
Vitiligo affects roughly 1% of the population. Research has shown that patients suffering from vitiligo are more likely to have a concomitant autoimmune thyroid disease such as Hashimotos thyroiditis or Graves disease, as well as other autoimmune disorders including type I diabetes, alopecia areata, psoriasis, rheumatoid arthritis, and Lupus.
What is important to remember is that when the hands are affected by the loss of pigmentation, thyroid disease is more likely to be present. In fact, research has suggested that patients with vitiligo who have lesions over their hands are 7 times more likely to have associated autoimmune thyroid disease than vitiligo patients who do not have skin loss of pigmentation in their hands.
If you have vitiligo, and especially if your hands are affected, you should be evaluated for an autoimmune thyroid condition and have thyroid testing done to determine if you have a thyroid dysfunction.

Molecular Mechanisms Underlying Vitiligo And Thyroid Autoimmune Disease Association
The reported association of vitiligo with AITD suggests the presence of shared heritable susceptibility genes . Thirty-seven susceptibility genes have been identified for vitiligo disease and more than 15 for AITD . Genome-wide linkage analysis and candidate gene association studies identified nine loci potentially involved in both AITD and vitiligo . Among these, there are organ-specific genes such as those coding for TYR, Tg, and TSHR . In addition, an autoimmunity susceptibility locus was identified by genome-wide linkage analysis on chromosome 1 in families characterized by vitiligo and Hashimotos thyroiditis . Among the 27 genes mapping to the AIS1 locus, the forkhead transcription factor D3 appears to be the most plausible responsible for the concomitant occurrence of vitiligo and AITD . In addition, a single nucleotide polymorphism of the PTPN22 gene, encoding a lymphoid specific phosphatase, is shared among patients with vitiligo and AITD . These findings suggest that the association observed between vitiligo and AITD could be explained, at least in part, by the sharing of a subset of susceptibility genes.
Hypothyroidism Vitiligo And Concurrent Treatment
According to research, hypothyroidism is the most common autoimmune illness linked with vitiligo. The report also covers the significance of screening for autoimmune disorders, and the importance of concurrent treatment in accomplishing ideal repigmentation.
A case was published of an 18-year-old woman with vitiligo accomplishing excellent repigmentation after getting thyroid replacement, oral corticosteroids, an oral anti-oxidant, and PUVA.
The relationship between vitiligo and hyperthyroidism was examined through the paper made by Colucci et al. A Closer Look On Vitiligo
According to this research published online, vitiligo is one of the top causes of acquired oral, skin, and hair depigmentation. It often appears as an inherited illness. It affects about 1% of the entire population.
Clinically, its described as the presence of distorting delineated skin macules that follow the destruction of melanocytes caused by auto-reactive cytotoxic T-lymphocytes.
Vitiligo is not a life-threatening disease but can potentially lead to skin cancer and other skin problems when the affected area is regularly exposed to UV light.
