Thyroid function tests are a set of blood tests used to measure how well your thyroid gland is working. They help detect an overactive or underactive thyroid gland and also give hints about other related conditions. There are three main types of thyroid blood tests:

  • TSH test: A thyroid stimulating hormone is not released by the thyroid gland. It is made in the pituitary gland in the brain. When thyroid levels in your body get low, the pituitary gland makes more TSH which serves as a signal for the thyroid gland to compensate and release more thyroid hormones. When thyroid levels are high, the pituitary gland makes less TSH. Hence, the TSH levels that are too high or too low can tell if your thyroid is working well.
  • T4 test: Measures the blood level of the hormone T4 .
  • T3 test: Measures the other major thyroid hormone T3 in the blood.

What About Fasting For A Blood Test During Pregnancy

There are several blood tests you may need if youre pregnant. These are designed to assess any potential health concerns that you or your baby might experience during pregnancy or after you give birth. Some of these tests will require you to fast beforehand. Your doctor will advise you how to prepare for each test.

Fasting is usually safe if youre pregnant, provided youre in good health and arent having a high-risk pregnancy. For your overall comfort, your doctor may advise you to drink extra water or to remain indoors, especially if the weather is very hot or humid.

Free Levels Vs Total Levels Of T3 And T4

If you noticed, the tests done in the study were free levels of T3 and T4. The cheaper test, and more commonly done, is total T4. Measuring the free levels of hormone in your blood means we are measuring how much is available to work on the cellular level.

Total levels of hormone are a combination of free levels and bound levels. You see, most of the hormones are bound by proteins to carry them around in the bloodstream, so they can be available at distant sites. When the free hormone is used by a cell, the binding protein can release another hormone so it is then available. The binding of the hormone is a complex deal that is affected by other circulating hormones, such as estrogen.

Symptoms That Might Require A Fasting Lab Test

You may have nonspecific symptoms that are hard to diagnose. Or, you may have the following symptoms that could prompt your doctor to order a fasting lab test:

  • Excessive urination and thirst could indicate diabetes.
  • Swollen feet and ankles could indicate kidney disease.
  • A high blood pressure reading could indicate high cholesterol.
  • A yellowish tint to your skin and eyes may indicate a liver condition.
  • Fatigue and an irregular heartbeat could indicate an electrolyte imbalance.
  • Pale skin and leg cramps could indicate anemia or a vitamin B12 deficiency.

Many other symptoms could make you need a fasting lab test. Your doctor might order a fasting lab test to rule out some of these conditions.

If youre getting your TSH levels checked to monitor your thyroid health, its best to get that done in the morning. Otherwise your results may come back normal even though you have hypothyroidism.

All the bodys hormones follow a daily rhythm, including thyroid hormone. This means there are times of the day when it naturally higher or lower. Researchers tested the blood of hypothyroid subjects both before 8 a.m. and again between 2 and 4 p.m.

In hypothyroid patients both untreated and on thyroid medication, TSH dropped was substantially lower during the afternoon test. This means an estimated 50 percent of people with hypothyroidism are not being diagnosed.

In the untreated group, TSH was 5.83 in the morning and 3.79 in the afternoon. In the treated group, TSH was 3.27 in the morning and 2.18 in the afternoon.

A 2004 study also showed late morning, non-fasting TSH was 26 percent lower compared to early morning, fasting TSH. This means even a late morning blood draw could result in a failure to diagnose.

Why Would I Need These Tests

For many years, people usually did their routine blood tests on an empty stomach. Inevitably, this caused a major congestion in the hospital every morning as people lined up at the blood testing centre in a queue that would spiral around corners and down hallways as people waited, sometimes for hours, to get tested.

Getting your routine blood tests on an empty stomach was important mainly because of the tests to check blood sugar and cholesterol. Most other blood tests, like hemoglobin levels, renal function, liver function, thyroid hormones, sodium and potassium levels dont need to be done on an empty stomach because they do not change before or after meals to any meaningful degree.

Blood sugar, of course, does change with meals and will often spike after you eat something, especially something very sugary or starchy. So, for many years, physicians used fasting blood sugar levels to test for and follow-up on diabetes. There are a few problems with testing blood sugar levels, and over the past few years, this has largely been replaced by testing Hemoglobin A1c . HbA1c gives you a three-month average of your blood sugar control and is therefore immune from the fluctuations that we often see with single blood sugar measurements.
