Q.Sometimes my doctor tells me it’s okay to eat before a blood test, and sometimes it isn’t. Why is that?

A. Actually, fasting affects the results of very few blood tests. For example, measurements of kidney, liver, and thyroid function, as well as blood counts, are not influenced by fasting. However, fasting is required before commonly ordered tests for glucose and triglycerides for accurate results.

    The timing of your blood draw plays an important role in reading a thyroid panel. However, there is more to it.

    Even with an early morning blood draw, many doctors will still fail to diagnose hypothyroidism because they use lab ranges that are too wide and that do not reflect genuine thyroid health.

    Many doctors still use a hypothyroidism TSH range of 0.5 to 5.0 even though the American Association of Clinical Endocrinologists recommends 0.3 to 3.0.

    In functional medicine we use an even narrower range of 1.8 to 3.0. We also know that only looking at TSH can miss hypothyroidism.

    For example, TSH may be normal but other thyroid markers are off. Thats why its important to order a thyroid panel that includes total and free T4 and T3, reverse T3, free thyroxine index , T3 uptake, and thyroid binding globulins. Many conditions can cause poor thyroid function, including inflammation, hormonal imbalances, and chronic stress. Ordering these other thyroid markers provides more insight into such imbalances.

    When Should I Get A Tsh Test

    TSH is often the first test doctors order when they suspect a patient has a thyroid disorder. Both hypothyroidism and hyperthyroidism are common disorders. All patients with symptoms of these thyroid disorders should have their TSH checked.

    If you are having symptoms of these disorders, your doctor may order a TSH or a thyroid panel test. Symptoms of hypothyroidism include:

    The two hormones, thyroxine and thyroid-stimulating hormone , work together and are usually in balance. In a healthy person the brain produces just the right amount of TSH to keep the thyroid gland ticking over. The thyroid gland then produces just the right amount of thyroxine.
