    What Causes A Goiter

    One of the most common causes of goiter formation worldwide is iodine deficiency. While this was a very frequent cause of goiter in the United States many years ago, it is no longer commonly observed. The primary activity of the thyroid gland is to concentrate iodine from the blood to make thyroid hormone. The gland cannot make enough thyroid hormone if it does not have enough iodine. Therefore, with iodine deficiency the individual will become hypothyroid. Consequently, the pituitary gland in the brain senses the thyroid hormone level is too low and sends a signal to the thyroid. This signal is called thyroid stimulating hormone . As the name implies, this hormone stimulates the thyroid to produce thyroid hormone and to grow in size. This abnormal growth in size produces what is termed a goiter. Thus, iodine deficiency is one cause of goiter development. Wherever iodine deficiency is common, goiter will be common. It remains a common cause of goiters in other parts of the world.

    Hashimotos thyroiditis is a more common cause of goiter formation in the US. This is an autoimmune condition in which there is destruction of the thyroid gland by ones own immune system. As the gland becomes more damaged, it is less able to make adequate supplies of thyroid hormone. The pituitary gland senses a low thyroid hormone level and secretes more TSH to stimulate the thyroid. This stimulation causes the thyroid to grow, which may produce a goiter.

    Diagnosis Of Thyroid Disease

    The vast majority of thyroid nodules are non-cancerous, but in order to determine whether or not the nodule is malignant, a doctor familiar with this field will have to run tests to make a diagnosis.

    Ultrasound is the most common imaging tool for thyroid nodules, along with a fine-needle aspiration biopsy, which provides a tissue sample for testing whether the nodule is malignant. Because only about 10 percent of nodules turn out to be malignant, the decision to operate on patients is only made after diagnosis with ultrasonography.

    An ultrasound will identify nodules that are too small to be detected by a simple neck examination and the presence of multiple nodules. Plus, an accurate measurement of the nodules can be taken with ultrasonography.

    If the results of an ultrasound call for a fine-needle aspiration biopsy, its usually performed with ultrasound guidance. A health care provider will take cells from the thyroid nodule and examine it under a microscope to detect whether its cancerous or benign.

    Performing a biopsy to determine the risk of malignancy has dramatically decreased the number of thyroidectomies performed. If the biopsy shows a benign lesion, it will be followed with yearly ultrasound examinations and repeated fine-needle aspiration if the nodule increases in size or development. If the lesion is malignant, total thyroidectomy is recommended.

    Goiters Nodules And Thyroid Storms

    Goiters and nodules can sometimes be a little tricky.

    As the thyroid gland becomes enlarged with too much thyroid hormone , you can run into issues where the thyroid gland unloads too much thyroid hormone at one time.

    This is called a thyroid storm.

    You see, this buildup of thyroid hormone in the gland can make you very hypothyroid because your gland isnt releasing much thyroid hormone.

    Then, all of a sudden, your gland can unload thyroid hormone very quickly.

    This large amount of thyroid hormone being secreted very quickly can put you into what we call a transient or temporary hyperthyroid state.

    We generally see symptoms such as:

    • Elevated heart rate
    • Feelings of extreme stress

    Often, thyroid storms occur in a cyclical pattern.

    Its a cycle where you transition between symptoms of hypothyroidism and hyperthyroidism as your thyroid gland is blocked and enlarges , then unloads before becoming blocked again, starting the process all over.

    We have clients who have been misdiagnosed as hyperthyroid because they were seen by their doctor during an unloading phase, when hyperthyroid symptoms are often present.

    This can lead to unnecessary medical treatments such as thyroidectomy or Radioactive Iodine treatment, both of which cause permanent damage to the thyroid gland.

    Yet, the important thing to realize is that you are actually hypothyroid.

    In reality, this is just a transient or temporary hyperthyroid condition.

    • Eat several small meals instead of a heavy meal. This will allow the body to better absorb all the nutrients.
    • Limit your intake of goitrogenic foods, such as radishes, sweet potatoes, strawberries, spinach, broccoli, peanuts, turnips, cabbage, cauliflower, and soybeans.
    • Avoid processed foods, soy products, and beverages such as coffee, alcohol, and soda.
    • Switch to iodized salt instead of regular salt.
