What Is Radiofrequency Ablation
Radiofrequency ablation is a technique using an electrode with high frequency alternating current to cause thermal injury in soft tissue leading to cell death and shrinking of the affected part.
RFA technology has been widely used for a range of conditions, including nerve pain, certain types of cancer, uterine fibroids, and some vascular conditions. This technology was approved by the U.S. Food and Drug Administration to be used for benign thyroid nodules in 2018.
What The Thyroid Gland Does
To understand why some types of goiter develop, it is first important to know what the normal function of the thyroid gland is and how it is regulated. The thyroid gland makes and releases into blood two small chemicals, called thyroid hormones: thyroxine and triiodothyronine . Each of them is comprised of a pair of connected tyrosine amino acids to which four or three iodine molecules, respectively, are attached.
The iodine needed for thyroid hormone production comes from our diet in seafood, dairy products, store bought bread, and iodized salt. Once absorbed, iodine in blood is trapped by a special pump in thyroid cells, called the sodium-iodide symporter. The thyroid also has several specialized biochemical ‘fastening machines,’ called enzymes, that then carry out the steps needed to attach iodine to particular parts of a very big protein called thyroglobulin, which is made only by thyroid cells.
Some of this thyroglobulin with iodine molecules attached is stored in the gland in the form of a gooey paste called colloid, which is normally located in the center of follicles, which are balls of thyroid cells with a hollow center.

There are three tests that may be considered in any patient with a thyroid nodule. The first is a blood test in order to determine how much thyroid hormone is being produced. Thyroid nodules rarely produce too much thyroid hormone. But when excessive thyroid hormone is being produced by the thyroid nodule this is almost always associated with a benign nodule. Benign thyroid nodules that produce extra thyroid hormone are usually removed to cure the excessive hormone production.

Shrinking the thyroid gland involves promoting the release of thyroid hormone from your thyroid gland slowly.

The most effective way to shrink a goiter is through the use of progesterone.
Just as estrogen blocks your thyroid gland from releasing thyroid hormone, progesterone does the opposite it activates the proteolytic enzymes that release the thyroid hormone.
Ordinarily, this is a very good thing, because we want to promote the thyroid gland to release thyroid hormone properly.

What About Radiation Exposure
Ionizing radiation has been known for a number of years to be associated with a small increased risk of developing thyroid cancer. The risk is very small and the amount of radiation exposure is higher than that produced by the routine use of x-rays. There is typically a delay of 5 years or more between radiation exposure and the development of thyroid cancer.Radiation was used occasionally between the 1920s and 1950s to treat certain head and neck conditions, such as enlarged tonsils, certain skin conditions , and sinusitis.Testing for the possibility of thyroid cancer developing from exposure to radiation has been done in the United States. These epidemiological studies determined that nuclear tests would likely increase the number of thyroid cancers seen in Americans over future decades.
However, the risks are greatest for those patients living near test sites for many years. If there is any good news from this report, it is that these cancers will typically be of the well-differentiated papillary thyroid cancer, the type that has an excellent prognosis with a very high cure rate. Despite these increased risks, thyroid cancer is still relatively uncommon and usually very curable.
Inherited types of thyroid cancer are very uncommon. The one type of thyroid cancer that may have a higher genetic link, by far, is called medullary thyroid cancer. Approximately only 3% of all thyroid cancers are medullary thyroid cancer and of these, less than one half are inherited.
What Is Molecular Profiling
At UCLA, thyroid nodules with indeterminate biopsies are sent out for an additional molecular marker test. An indeterminate biopsy result is the gray zone where the risk of cancer is intermediate but cannot be ignored.
Sometimes the biopsy result is reported as indeterminate. This means the cells are not normal, but there are not definite signs of cancer. When biopsies are indeterminate, the risk of thyroid cancer is 15-30%.
In the past, to avoid missing a cancer, we recommended thyroid lobectomy to establish a definitive diagnosis. Now, we use molecular profiling. This refers to commercial DNA or RNA tests made specifically for indeterminate thyroid nodules. If the genetic profile appears benign, patients can avoid surgery and we simply watch the nodule over time with neck ultrasound.

Are There Alternatives To Surgery For Goiter
