Sometimes the thyroid begins to grow , causing one or more nodules to form. Why this happens is not known. Cancer is the biggest concern when nodules form. Fortunately, cancer is very rare it is found in less than 5 percent of all nodules. Nodules develop more often in people who have a family history of nodules, and in people who dont get enough iodine. Iodine is needed to make thyroid hormone.

There are different types of thyroid nodules:

• Colloid nodules: These are one or more overgrowths of normal thyroid tissue. These growths are benign . They may grow large, but they do not spread beyond the thyroid gland.
• Thyroid cysts: These are growths that are filled with fluid or partly solid and partly filled with fluid.
• Inflammatory nodules: These nodules develop as a result of chronic inflammation of the thyroid gland. These growths may or may not cause pain.
• Multinodular goiter: Sometimes an enlarged thyroid is made up of many nodules .
• Hyperfunctioning thyroid nodules: These nodules autonomously produce thyroid hormone without regard for normal feedback control mechanisms, which may lead to the development of hyperthyroidism. Hyperthyroidism can affect the heart and cause such problems as sudden cardiac arrest, high blood pressure, arrhythmias , osteoporosis and other health problems.
• Thyroid cancer: Less than 5 percent of thyroid nodules are cancerous.

What Is Molecular Profiling

At UCLA, thyroid nodules with indeterminate biopsies are sent out for an additional molecular marker test. An indeterminate biopsy result is the gray zone where the risk of cancer is intermediate but cannot be ignored.

Sometimes the biopsy result is reported as indeterminate. This means the cells are not normal, but there are not definite signs of cancer. When biopsies are indeterminate, the risk of thyroid cancer is 15-30%.

In the past, to avoid missing a cancer, we recommended thyroid lobectomy to establish a definitive diagnosis. Now, we use molecular profiling. This refers to commercial DNA or RNA tests made specifically for indeterminate thyroid nodules. If the genetic profile appears benign, patients can avoid surgery and we simply watch the nodule over time with neck ultrasound.

How To Do A Thyroid Self

Below are easy steps for examining your thyroid:

1. Face a mirror

Position yourself or the mirror to focus on the lower-front portion of your neck, and remove anything that could block the view of your neck. â

2. Tilt your head

Extend your neck back slightly, pointing your chin toward the ceiling, while still being able to see into the mirror without trouble.â

3. Find your thyroid

Put your finger on tip of your chin and slide that finger down the midline. The first structure you hit is the top of the thyroid cartilage, which despite its name, is not where the thyroid gland is situated. Keep moving your finger down your neck to the Adam’s apple. Just beyond you will feel the cricoid cartilage. Continue downward below the cricoid ring are the first two rings of the trachea on top of which lies the thyroid gland.

4. Take a sip of water

Keeping your neck extended backward, take a sip of water and swallow. â

5. Look for lumps

As you swallow, with your neck still extended back, examine your neck for any visual irregularities including enlargements, lumps, protrusions, or imbalances. â

6. Touch the area

Gently feel the area around your thyroid for any enlargement or lumps. Thyroid nodules usually appear round in shape and move/roll with the gland when you swallow. A goiter can be found on one or both sides of the thyroid.

7. Repeat the process.

Do the tilt-swallow-look process a few times to observe the structures in your neck.â

Other Causes Of Hypothyroidism

In some cases, hypothyroidism results from a problem with the pituitary gland, which is at the base of the brain. This gland produces thyroid-stimulating hormone , which tells the thyroid to do its job. If your pituitary gland does not produce enough TSH, levels of thyroid hormones will fall. What Does A Thyroid Fine Needle Aspiration Or Biopsy Entail

In some situations this is performed with local anesthesia in the clinic. Your child will be awake. In very young children the FNA is done in the operating room under general anesthesia. The pathologist will look at the tissue to determine what they thyroid nodule is composed of and if additional workup or surgery is needed. It often takes about one to two weeks to get the results.

Your Thyroid Does Change As You Age

These tests can be done on tissue taken during a biopsy or surgery for thyroid cancer. If the biopsy sample is too small and all the molecular tests cant be done, the testing may also be done on blood that is taken from a vein, just like a regular blood draw.
