Understand More About Your Weight Gain

Your weight loss problems arent just due to a slower metabolism. When you have an underactive thyroid, your body will start to retain salt and water. This will give the impression that youre gaining weight and it will be much easier to get rid of them once your medication is balanced.

Its so important to understand as much as possible about your hypothyroidism and your weight gain. Know why its happening, so you can take steps to get the weight off afterwards.

Salt and water retention will disappear, but other reasons for weight gain wont. Reassess your diet and your exercise plan. Are you really eating as healthy as you possibly could? Is it possible to go on a diet to help shed some of the pounds?

Be completely honest with yourself. Dont worry about what other people have said to you or what your doctor things right now. You want to do the best thing possible for your own life and your own mindset.

Remember that the only person you are letting down by lying is yourself. The Thyroid/weight Gain Connection

Hypothyroidism has long been associated with weight gain , but the exact biochemical cause of this link is not completely clear. That said, there are several mechanisms that may explain the connection in cases of low thyroid function.

The two most active thyroid hormones, thyroxine and triiodothyronine , circulate in the body, and they affect your metabolism through their interaction with your:

  • Fat cells
  • Pancreas
  • Hypothalamus

Thyroid hormones normally help the body break down fat, and they help the liver and pancreas function to metabolize stored calories to be used for energy. These hormones also help the muscles throughout the body as they use energy. And when there is an adequate amount of thyroid hormones circulating in the body, the hypothalamus, which is a regulator of thyroid hormone in the brain, decreases the amount of thyrotropin regulating hormone secretion.

Treatment with thyroid replacement medications does not necessarily induce weight loss, even when optimal thyroid hormone levels are measured on blood tests.

What Is The Relationship Between Hyperthyroidism And Weight

Since the BMR in patients with hyperthyroidism is elevated, many patients with an overactive thyroid do, indeed, have some weight loss. Furthermore, weight loss is related to the severity of the overactive thyroid. For example, if the thyroid is extremely overactive, the individuals BMR increases which leads to increased calories needed to maintain the body weight. If the person does not increase the amount of calories eaten to match the excess calories burned, then there will be weight loss. As indicated earlier, the factors that control our appetite, metabolism, and activity are very complex and thyroid hormone is only one factor in this complex system. Nevertheless, on average the more severe the hyperthyroidism, the greater the weight loss observed. Weight loss is also observed in other conditions where thyroid hormones are elevated, such as in the toxic phase of thyroiditis or if the dose of thyroid hormone pills is too high for a patient. Since hyperthyroidism also increases appetite, some patients may not lose weight, and some may actually gain weight, depending on how much they increase their caloric intake.

Consider Medication Side Effects

As a consultant pharmacist, I have witnessed numerous people having major changes in weight gain with the use of certain medications.

Birth control, in particular, can be associated with significant weight gain, and 45 percent of my readers with Hashimotos reported that stopping birth control helped them normalize their weight. Ive written about the impact of birth control on our health in this article.

As people with Hashimotos are often misdiagnosed with anxiety, depression, bipolar disorder, etc. I would be remiss to report the important impact of psychotropic medications on weight gain.

The biggest culprits associated with weight gain include antipsychotic medications which were traditionally developed for schizophrenia but are used on and off label for conditions like bipolar disorder, depression, anxiety, irritability and even insomnia! In my experience, Zyprexa was the worst offender. I have seen people gain 100+ pounds on this medication. Other antipsychotics include Seroquel, Risperdal, and Abilify.

Antidepressants such as Prozac, Zoloft and Lexapro can also contribute to weight gain. The antidepressant Wellbutrin, however, is less likely to cause weight gain.

I recommend speaking with your doctor or pharmacist about your current medications to see if they could be potential culprits for causing weight gain, and if weight neutral alternatives exist.

Reduce Simple Carbs And Sugars

Complex carbohydrates found in whole foods, such as certain fruits and vegetables, whole grains, and legumes, take longer to digest and are a more stable source of energy compared with simple sugars, such as candy, white bread, sugary cereal, syrups, and fruit juice concentrate. A moderate to low carbohydrate diet that focuses on complex carbohydrates and avoids simple sugars can help you to lose weight by keeping you fuller for longer and regulating your blood sugar levels. You should also avoid foods that cause inflammation in the body.

A low glycemic diet that focuses on non-starchy vegetables, whole grains, beans, some fruits, and healthy proteins and fats, can help you control your weight by digesting foods at a slower rate, meaning you remain fuller for longer and minimize spikes in blood sugar and insulin levels.

Thyroid Function In Obese Subjects

TSH levels are at the upper limit of the normal range or slightly increased in obese children, adolescents, and adults and are positively correlated with BMI. Low fT4 with a moderate increase in T3 or free T3 levels has been reported in obese subjects. Progressive fat accumulation was associated with a parallel increase in TSH, and fT3 levels irrespective of insulin sensitivity and metabolic parameters and a positive association has been reported between the fT3 to fT4 ratio and both waist circumference and BMI in obese patients. Although the typical picture of high TSH, low fT4, and high fT3 is the most common, various studies on adult obese individuals report thyroid hormone and TSH concentrations as normal, elevated, or reduced.

In obese children, the most common abnormality clearly is hyperthyrotropinemia. Recently, it has also been shown that obese pediatric patients frequently have an ultrasound pattern of the thyroid which is highly suggestive of Hashimoto’s thyroiditis. These findings are not associated, however, with production of thyroid autoantibodies.

What Is The Relationship Between Hypothyroidism And Weight Gain

Since the BMR in the patient with hypothyroidism is decreased, an underactive thyroid is generally associated with some weight gain. The weight gain is often greater in those individuals with more severe hypothyroidism. However, the decrease in BMR due to hypothyroidism is usually much less dramatic than the marked increase seen in hyperthyroidism, leading to more modest alterations in weight due to the underactive thyroid. The cause of the weight gain in hypothyroid individuals is also complex, and may not be related to excess fat accumulation. Most of the extra weight gained in hypothyroid individuals is due to excess accumulation of salt and water. Massive weight gain is rarely associated with hypothyroidism. In general, 5-10 pounds of body weight may be attributable to the thyroid, depending on the severity of the hypothyroidism. Finally, if weight gain is the only symptom of hypothyroidism that is present, it is less likely that the weight gain is solely due to the thyroid.

There is clinical evidence suggesting that even mild thyroid dysfunction in the form of subclinical hypothyroidism is linked to significant changes in body weight and represents a risk factor for overweight and obesity however, this remains a gray area. It has been further noted that small variations in serum TSH caused by minimal changes in L-T4 dosage during replacement therapy are associated with significantly altered REE in hypothyroid patients. However, there is a paucity of data regarding the actual extent of weight gain and weight loss with L-T4 treatment in hypothyroidism.

Dr. Boelaert still cant say for sure why individuals who are treated with antithyroid drugs to reverse their hyperthyroidism show a tendency to gain back the lost weight plus more. “It appears something in the disease process or perhaps from the treatment makes them end up heavier.” Clearly, more research is needed for us to understand the specific mechanisms behind the shift upward in body weight as a result of non-surgical treatment for overactive thyroid, she says.

Also, we know that appetite signals get disturbed when the thyroid goes out of whack, she says. When patients come in for treatment, ”lots of patients say, ‘I am losing weight although I feel like am eating like a horse.'” After the antithyroid treatment begins, at least some people may continue to eat the same larger amounts that they have grown accustomed to, and this may contribute to the weight gain, she says.

Do appetite signals return to normal once treatment begin? “We don’t know, but we hope to study this,” Dr. Boelaert says.

She worries about the adverse effects that are associated with overweight. “People with an overactive thyroid already have an increased risk of cardiovascular disease,” she explains. “If they then become overweight or obese, that is a further ‘hit.”’ So recognizing your increased risk for heart disease, and concerns about weight gain should be discussed with your doctor, and incorporated into your treatment plan.

Get A Diagnosis As Quickly As Possible

If your thyroid is underactive, inadequate treatment may make weight loss almost impossible, despite diet and exercise. And the longer it takes you to get diagnosed, the more weight you could gain.

When you have mild hypothyroidism, even before your TSH is elevated enough to warrant treatment, your metabolism can slow down significantly, causing you to burn fewer calories each day. First and foremost, I am not a fan of restrictive calorie diets especially for anybody but especially for people with Hashimotos. One of the things that happens is when youre put on a low-calorie diet is youre going to be putting your body into basically adrenal dysfunction where your body is going to say, no, Im starving. Lets control everything. Lets slow down our metabolism. Lets prevent us from gaining weight From losing any weight and extending any calories.
