It has long been known that many foods and medications including calcium supplements, soy, and ironinterfere with the absorption of oral levothyroxine in the body.1 The plot thickens. For the first time, drinking cows milk is demonstrated to reduce absorption of levothyroxine,2 according to a study published in the journal Thyroid.

Levothyroxine is one of the most commonly prescribed medications in the US.3;;The findings from this prominent team of researchers at the UCLA David Geffen School of Medicine in California are particularly significant since patients are typically instructed to take levothyroxine an hour before breakfast, a meal that commonly includes dairy.

;Changes in the amount of levothyroxine absorbed can lead to changes in thyroid blood tests used to monitor the appropriateness of its dose, says lead author Angela M. Leung, MD, MSc, assistant professor of medicine at the UCLA David Geffen School of Medicine.

Treatment for hypothyroid must be timed to avoid milk.

Water is considered as the best beverage to take your medicines with. Fresh drinking water does not inhibit drug metabolism nor does it interfere with its quick absorption in the body. Other beverages like soft drinks or aerated beverages, coffee or tea, alcohol can often cause some drug interactions or allergies in the case of few patients. While milk is safe to be consumed with calcium or vitamin D tablets, it may not be as safe with all other medication. But should you be taking medicines with chilled or lukewarm water? Dr Rajni Pathak, Co-ordinator Preventive Health Check, Fortis Hospital Mohali tells us more about how the right temperature of the water is important while swallowing medicines.

Water basically acts as a medium of transport for your medicines. It prevents choking and also improves absorption of the drugs in your stomach and intestine.

The water should ideally be at room temperature. In case of a cough and throat infections, it is advised to take the medicine with lukewarm water. The temperature of water soothes the throat and even speeds the dissolving process. In fact, throat infection and cold medicines don t react adversely to the temperature of water. Also, the right technique of swallowing pills is to drink a few sips of water before taking the medicine so that your throat is not very dry and the tablets can be swallowed with ease.

Worth Considering A Bedtime Switch

Typically, patients are advised to take their levothyroxine first thing in the morning, at least 30 minutes but preferably an hour before eating, on an empty stomach and with only water.5; The goal is to achieve consistency in taking the medication to avoid fluctuations in thyroid levels and variable control of symptoms.

But for patients who may want to add milk to their morning coffee or cereal, there are alternative dosing strategies, including taking levothyroxine at night.

While not all patients may be candidates for bedtime administration,;those who have a consistent sleep routine and dont take other medications before bed might consider making this switch.

Of course, before you make any change in the timing of when you take your levothyroxine, you should speak to your doctor.; And, its important to realize that there must be four hours between your last meal or snack when you plan to take your levothyroxine at bedtime. If you are a nighttime eater, this is likely not a good change for you to make.

Why Is This Medication Prescribed

Levothyroxine is used to treat hypothyroidism . It is also used with surgery and radioactive iodine therapy to treat thyroid cancer. Levothyroxine is in a class of medications called hormones. It works by replacing thyroid hormone that is normally produced by the body.

Without thyroid hormone, your body cannot function properly, which may result in poor growth, slow speech, lack of energy, excessive tiredness, constipation, weight gain, hair loss, dry, thick skin, increased sensitivity to cold, joint and muscle pain, heavy or irregular menstrual periods, and depression. When taken correctly, levothyroxine reverses these symptoms.

Management Of Potential Competing Interests

As per ATA policy, the results of any reviews by the Ethics Committee were shared with the entire group, with an opportunity to voice any additional potential questions or concerns relating to the review.

There should be recognition that there are not enough data to resolve clinical disagreement among thyroid experts regarding treatment for hypothyroidism. Clinical equipoise is disturbed only by the results of well-designed randomized controlled trials that have the statistical power to settle the question of efficacy between monotherapy and combination therapy, or other forms of therapy.

In evaluating published research and original studies of TH replacement, only those studies demonstrating ethically sound design and responsible conduct of research , in which there was study oversight and Institutional Review Board approval, should be considered. Well-designed small studies without the statistical power to resolve clinical disagreement, or disturb clinical equipoise , cannot be considered to be a high level of evidence of a null effect or superior or inferior efficacy.
